Hayward Gardens Post Acute is seeking a compassionate, organized, and dependable Social Services Assistant to join our team full-time. In this role, you will work alongside the Social Services Department to help support residents and their families, ensuring they receive the resources, advocacy, and emotional support needed throughout their stay.

Candidates with prior experience in healthcare, skilled nursing, or social services are strongly encouraged to apply.

Position Details
  • Position: Full-Time

  • Pay: $25.00 per hour

  • Location: Hayward Gardens Post Acute

Responsibilities
  • Assist the Social Services Director with resident assessments and care planning.

  • Help coordinate admissions, discharges, and resident transitions.

  • Support residents and families by providing information on available community resources.

  • Maintain accurate documentation and resident records.

  • Assist with resident rights, grievance procedures, and advocacy efforts.

  • Collaborate with nursing, rehabilitation, and interdisciplinary teams to support resident well-being.

  • Help coordinate family meetings and care conferences as needed.

  • Maintain confidentiality while ensuring compliance with all federal and state regulations.

Qualifications
  • Previous experience in a skilled nursing facility, healthcare setting, or social services role is highly recommended.

  • Excellent communication and interpersonal skills.

  • Strong organizational skills with the ability to multitask.

  • Proficiency with computers and electronic medical records is preferred.

  • Compassionate, professional, and committed to providing exceptional resident care.

  • Ability to work effectively as part of a multidisciplinary team.
