JOB SUMMARY

A Speech Language Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their

place of residence. This is performed in accordance with physician orders and plan of care under the direction

and supervision of the Clinical Manager/Therapy Supervisor. Speech therapy services are furnished only by

or under the supervision of a qualified speech pathologist or audiologist.

Qualifications

QUALIFICATIONS

1. A person who has a masters or doctoral degree in speech-language pathology, and who meets either of the

following requirements.

a. Is licensed as a speech-language pathologist by the state in which the individual furnishes such services.

OR

b. In the case of an individual who furnishes services in a state which does not license speech-language

pathologists:

i. Has successfully completed 350 clock hours of supervised clinical practicum (or is in the process of

accumulating supervised clinical experience);

ii. Performed not less than 9 months of supervised full-time speech-language pathology services

after obtaining a masters or doctoral degree in speech-language pathology or a related field; and

iii. Successfully completed a national examination in speech-language pathology approved by the

Secretary.

2. Two (2) years experience, preferred.

3. Acceptance of philosophy and goals of Agency.

4. Ability to exercise initiative and independent judgment.

LINE OF AUTHORITY

Reports to the Clinical Manager/Therapy Supervisor

RESPONSIBILITIES

1. Assists the physician or intermediate care provider in evaluating level of function, helps develop the plan

of care and revise as necessary.

2. Prepares clinical and progress notes.

3. Advises and consults with the family and other agency personnel.

4. Participates in in-service programs and presents in-service programs as assigned.

5. Understands and adheres to established policies and procedures.

6. Participates with all other health care personnel in patient care planning.

7. Completes initial history and evaluation visit, notifies physician of patient needs and submits plan of care

and orders for physician approval.

8. Performs all skilled Speech Language Pathology services as ordered by physician.

9. Consults with physicians regarding change in treatment.

10. Writes reports to physicians regarding patients progress.

11. Attends all staff meetings and patient care conferences as scheduled.

Skills Required

  • Master's or doctoral degree in speech-language pathology
  • Licensed as a speech-language pathologist by the state where services are furnished
  • If state does not license SLPs: 350 clock hours of supervised clinical practicum (or in process)
  • If state does not license SLPs: Minimum 9 months of supervised full-time SLP services after degree
  • If state does not license SLPs: Successful completion of a national examination in speech-language pathology approved by the Secretary
  • Two (2) years experience
  • Acceptance of the agency's philosophy and goals
  • Ability to exercise initiative and independent judgment
