GENERAL STATEMENT OF RESPONSIBILITIES:

Provides case management services to families referred by the Department of Children and Families (DCF) in collaboration with The Connection, Inc. Conducts intake assessments, develops individualized care plans, coordinates housing and community resources, and connects families to medical, behavioral health, financial, and social services. Monitors client progress through home visits and collateral contacts, collaborates with DCF social workers to promote child safety and family well-being, maintains accurate client records, prepares reports, and submits monthly activity statistics.

SUPERVISION: Supervision is provided in staff meetings, case review, and through direct observation.

Essential Duties and Responsibilities include the following:


  1. Adheres to Wellmore Behavioral Health's Code of Ethics and all agency policies.
  2. Conducts intake assessments, determines program eligibility, and develops, monitors, and updates individualized service plans with client participation.
  3. Provides comprehensive case management, including care coordination, referrals, advocacy, crisis intervention, supportive counseling, and linkage to housing, behavioral health, medical, educational, vocational, financial, and community resources.
  4. Collaborates with DCF social workers, treatment providers, landlords, and other community partners to coordinate services, ensure child safety and family stability, and secure appropriate housing placements.
  5. Conducts home visits, including unannounced visits and on-call responsibilities, to monitor client progress and compliance with service plans.
  6. Maintains accurate, timely, and confidential client documentation, prepares required reports, and participates in case reviews.
  7. Attends required trainings, supervision, and staff meetings at Wellmore Behavioral Health and The Connection, Inc.
  8. Performs other related duties as assigned.
Qualifications

QUALIFICATIONS:

EXPERIENCE:

Bachelor's Degree in a human services-related field or relevant experience in human services, preferably encompassing experience in child welfare, substance abuse, welfare or the supportive housing system. Will consider other combinations of education and experience.

LICENSURE/CERTIFICATION/REGISTRATION

Must have own transportation, valid drivers license and valid auto insurance; CPR and First Aid Certification

Skills Required

  • Bachelor's degree in a human services-related field or equivalent relevant experience
  • Experience in human services/case management
  • Experience in child welfare, substance abuse, welfare, or supportive housing system
  • Own transportation, valid driver's license, and valid auto insurance
