Job Description
Serves as clinical expert liaison for System Hospitals on activities related to core measures including data abstraction and entry, identification of opportunities for improvement, education, and development of related tools. Acts as a resource to sites by providing information and facilitating services.
Job Responsibility
- Abstracts pertinent information from medical records and enters it into the appropriate databases (Premier, Midas, GWTG, etc.).
- Notifies Quality Improvement Specialist when abstraction is completed.
- Follows-up to ensure correction of identified problems/issues.
- Assists in the development and revision of all measure related tools.
- Educates hospital staff in all aspects of core measures.
- Advises others on how best to meet requirements.
- Shares best practices with the hospitals.
- Identifies and reports possible performance improvement, patient safety, and complications.
- Maintains current knowledge of CMS, Joint Commission, Department of Health and GWTG requirements and supporting information system applications as it relates to the various measures.

Job Qualification
- Bachelor’s Degree in Nursing (BSN) required.
Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
- 2-5 years of technical experience, required.
- One-three (1-3) years clinical experience including general knowledge of QM principles, NYPORTS and JCAHO requirements, preferred.
- 3+ years of OB experience, highly preferred
