Days/Hours: Monday to Friday, 9am to 5pm

Fortune has a dual mission: both advocacy and service. The advocacy portion of our mission focuses on building a more just criminal legal system and reducing the barriers to reentry into society. The service side of Fortune has evolved to encompass a broad range of programs that started with employment and education and now includes a robust array of services, among them: alternatives to incarceration, supervised release, court advocacy, discharge planning within correctional facilities, licensed substance use and mental health treatment, a housing continuum ranging from emergency and transitional through permanent congregate and scattered site housing, assistance in obtaining benefits, a care management unit, HIV services, a food and nutrition program, and an arts program. Each service we provide is informed by the needs of the population we serve. Fortune currently serves approximately 18,000 people a year. Our program models are recognized, both nationally and internationally, for their quality and innovation. We have locations in Brooklyn, The Bronx, Manhattan and Queens. 

 

Position Summary:  

The Senior Case Manager will participate in and supervise the screening and assessments of formerly incarcerated clients with histories of mental illness, substance use, and/or HIV/AIDS in permanent housing. This role is also responsible for direct staff supervision, including assigning work, monitoring performance, providing coaching and training, supporting professional development, and ensuring supervised staff consistently meet program, documentation, and service-delivery expectations.

  • Clinical Assessment and Service Planning: Demonstrates the ability to assess client needs, identify risks, and support individualized service plans for individuals with mental health, substance use, HIV/AIDS, criminal justice, and housing-related needs. 
  • Supervision and Team Leadership: Provides effective supervision, coaching, and support to case management staff while promoting accountability, professional development, and consistent service delivery. 
  • Collaboration and Client Advocacy: Builds strong internal and external partnerships to connect clients with benefits, housing resources, treatment providers, and supportive services that promote stability and successful community reintegration. 

 

Essential Duties and Responsibilities:  

  • Review intake interviews and needs assessments of newly released individuals and clients eligible for transitional housing services;
  • Review of progress notes to ensure MOCJ Scope of Work is being met; 
  • Create individualized treatment plans for each client. Document all client services, including successful and unsuccessful attempts; 
  • Perform crisis intervention with clients as needed; 
  • Effectively collaborate internally within all Fortune Society departments to ensure ongoing communication and quality care. Follow up with referral agencies regarding client progress;   
  • Tracking new referrals and following up with intake in conjunction with Program Director; 
  • Provide direct supervision to case management staff, including assigning and monitoring work, reviewing documentation and service delivery, conducting regular check-ins, supporting training and professional development, addressing performance concerns, and carrying a small caseload of three to four participants, if necessary;
  • Ensure supervised staff comply with agency policies, funder requirements, documentation standards, case management protocols, and participant engagement expectations;
  • Oversee a team of staff, including weekly individual supervision; support staff in their professional development; complete six-month and annual evaluations to ensure quality professional standards are met; and address performance and discipline issues when necessary; 
  • Ensure that case managers assist clients in obtaining all necessary documentation to facilitate benefits application process. Complete and submit clients’ application to appropriate benefits provider agency (e.g., HRA, Social Security); 
  • Provide ongoing counseling and supportive services needed to achieve goals stated in client treatment plan;
  • Facilitate support groups and community meetings;
  • Enter all client services in the agency’s proprietary database, as well as in the appropriate contract funder databases, if applicable; 
  • Prepare and submit reports on client intake and discharge activities;
  • Ability to be on call 24 hours for emergencies and support is a must; 
  • Provide clinical expertise and support to the Director, as needed;
  • Perform other related duties as assigned.
Qualifications

Qualifications: 

  • MSW required; LMSW preferred. At least two years of experience dealing with HIV/AIDS, substance abuse, and/or criminal justice issues;
  • Two years of supervisory experience in a residential setting preferred;
  • Possess strong follow-up, organizational, and leadership skills;
  • Possess strong written and verbal skills;
  • Experience using Microsoft Office Suite;
  • Able to encourage disenfranchised populations to seek and obtain social services needed;
  • Possess a valid driver's license and proven driving ability as demonstrated by previous driving record.

Skills Required
