Impact Connections is seeking a compassionate and student-centered School-Based Guide to support students in grades 9–12 through school-based partnerships. The School-Based Guide will provide student support, care coordination, resource navigation, and collaboration with families, school staff, and community partners to help address barriers impacting student success.
This is a part-time position with an on-site schedule of Tuesdays from 9:00 AM–4:00 PM and Wednesdays from 9:00 AM–12:00 PM. The role also requires availability for remote meetings twice weekly with leadership and the Impact Connections team.
Key Responsibilities:
Provide individualized student support and check-ins
Assist students with goal-setting, problem-solving, and self-advocacy
Connect students and families to school-based and community resources
Support care coordination and follow-up with students, families, and partners
Maintain timely and professional documentation
Collaborate with school staff and Impact Connections leadership
Participate in meetings, supervision, and professional development
Requirements
Required Qualifications:
Certified Social Worker (CSW) in the state of Louisiana required
MSW from an accredited program required
Minimum of 2 years of experience working with adolescents in school, community-based, behavioral health, or youth-serving settings
Strong communication, organizational, and relationship-building skills
Experience supporting youth with social, emotional, behavioral, or resource-related needs
Preferred Qualifications:
Experience in school-based or youth-serving environments
Experience with care coordination, case management, or resource navigation
Familiarity with trauma-informed and restorative practices
Bilingual a plus

Skills Required
