Job Overview:
Joseph J. Peters Institute, a nationally recognized provider serving children, adolescents, and adult survivors of trauma, as well as perpetrators of sexual abuse, seeks a case manager coordinator in our Survivor Services department. Major responsibilities of this position: Monitors and manages databases, tracking systems, and progress note completion; assists with case assignments; audits charts; and oversees day-to-day support activities for therapists and supervisors. This position reports to the Operations Manager.
Responsibilities:
- Maintain the highest quality case management services to participants at JJPI.
- Schedules evaluations for evaluators and follows up with referral sources.
- Completes intake paperwork for participants scheduled for evaluations. Develop and implement innovative techniques to serve participants more effectively.
- Ensure compliance with management and stakeholder targets.
- Monitor and manage databases and tracking systems for evaluations, assigned cases, and discharged participants.
- Schedule participants for psychiatric services as needed and act as a liaison to therapists, evaluators, and psychiatrists.
- Lead ongoing quality management of Safety and Responsibility Program services. Specifically, lead ongoing chart audit, treatment plan tracking, and compliance with progress note completion parameters under the guidance of the Operations Manager.
- Ensure that psychiatric signatures are obtained for resiliency plans by the required deadlines.
- Train new hires on Credible usage, case management documentation requirements, all administrative processes and procedures, and time frames for completion.
- Collects and disseminates evaluation and wellness plan tracking to all clinicians weekly.
- Provide leadership, guidance, and supervision to case management/co-op intern staff at the Safety and Responsibility Program.
- Managing and monitoring referrals to ensure case managers/intern co-op interns are following up with them
- Initiate all medical records requests with outside stakeholders.
- Schedules, organizes documentation, and makes reminder calls as needed.
- Maintains up-to-date tracking of all caseload assignments (groups and individuals) for the Safety and Responsibility Program.
Skills:
- Ability to meet timelines.
- Ability to communicate in writing and orally.
- Good analytical and troubleshooting skills.
- Working knowledge of PowerPoint, Excel, and SPSS preferred.
Experience:
- Experience with human services systems.
- Knowledge of resources available to staff and participants that enhance, coordinate, and complement the services provided at JJPI.
- Two to three years of case management and operations/administrative experience.
Education Requirement:
- A Bachelor’s Degree in a field related to social services and/or clinical work is required.
Salary:
- Commensurate with education and experience
- Grade 16
