Day Shift - 7.5 Hours (United States of America)

The RN Care Coordinator is responsible for the continuity of patient care across the Amboy Medical Practice and supports the clinical and quality performance goals of the practice.  The RN Care Coordinator routinely assesses patients for readiness to learn, barriers to care, social determinants of health and other psychosocial behaviors that impact care.  The RN Care Coordinator is a resource to providers and clinical staff and ensures that the practice understands and meets the needs of its patients through pre-visit planning, daily huddles, referral and test tracking a care coordination services.

Requirements:

NYS RN license

Current BLS

Minimum two or more years of ambulatory, clinic or physician practice care experience required, preferably in a recognized Patient Centered Medical Home or similarly recognized program. 

Shift: Mon-Fri 8:30-4:30 eo Saturday 9-5

Salary Range: $59.67/hr - $64.22/hr (Commensurate with Experience)

Skills Required

  • New York State Registered Nurse license
  • Current Basic Life Support certification
  • At least two years of ambulatory, clinic, or physician practice care experience
  • Experience in a recognized Patient Centered Medical Home or similar program
