Make a Difference Every DayBig Bend Hospice is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our interdisciplinary hospice care team. In this rewarding role, you will coordinate and provide high-quality end-of-life care, helping patients and families achieve the best possible quality of life through expert symptom management, education, advocacy, and support.
Position SummaryThe RN Case Manager develops, coordinates, and implements the interdisciplinary plan of care for assigned hospice patients across various care settings. This role is responsible for patient assessments, care planning, symptom management, patient and family education, and collaboration with physicians and the hospice interdisciplinary team. The RN Case Manager also supervises Hospice Aides and LPNs while ensuring compliance with hospice regulations, nursing standards of practice, and Big Bend Hospice policies and procedures.
Key Responsibilities- Manage a hospice caseload of approximately 18–25 patients.
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Coordinate and oversee care delivered by the interdisciplinary team.
- Provide expert pain and symptom management.
- Educate patients, families, and community partners regarding hospice care.
- Maintain timely and accurate documentation within the electronic medical record (EMR).
- Collaborate with physicians and interdisciplinary team members to ensure quality care.
- Participate in quality improvement initiatives and interdisciplinary team meetings.
- Assist with orientation and mentoring of new staff and students
Work Schedule:
- Monday-Friday; 8:00 am- 5:00 pm; Rotating Holidays
Travel Requirement: You must have a reliable vehicle and be willing to be dispatched throughout our service areas.

Required:
- Current Florida Registered Nurse (RN) license.
- Minimum of one year of inpatient nursing experience, hospice, home health, and/or long-term care experience.
- Valid Florida driver's license, reliable transportation, and current auto insurance.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating effectively within a team environment.
- Proficiency with Microsoft Office and electronic health records.
- Passion for serving hospice patients and their families.
- Strong clinical assessment and critical-thinking skills.
- Compassionate bedside manner and excellent customer service.
- Commitment to professional growth and clinical excellence.
- Ability to manage the emotional and physical demands of end-of-life care
