Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital. 

RUTLAND CLINIC:

Community Health Rutland and Rutland Express Care serve the majority of those living in and around Rutland, the county seat and population center of southern Vermont. The Rutland practice uniquely provides the community with access to Primary Care, Behavioral Health and Care Management services for the whole family. Primary Care services include on-site Behavioral Health providers, Social Workers, and Care Management, along with preventive programs like diabetes management and education. Rutland Express Care, located within the same facility as Community Health Rutland, is open seven days a week for the treatment of minor illnesses and injuries for adults and children. 

Downtown Rutland, the third largest city in Vermont, is the county seat and home to Rutland Regional Medical Center. The Green Mountain National Forest, Killington, and Pico ski resorts are a short drive from the picturesque historic downtown area.

KNOWLEDGE,EDUCATION AND SKILL REQUIREMENTS:  


POSITION SUMMARY:

Community Health is seeking a Family Practice Physician to work at our clinic in Rutland, Vermont.  Physician candidates are expected to be knowledgeable and skilled in the delivery of primary care. Responsibilities include, but are not limited to:  

  • Evaluate, diagnose, and treat patients with rheumatic, autoimmune, inflammatory, and musculoskeletal disorders in accordance with current standards of care.
  • Order, interpret, and integrate laboratory testing, imaging studies, bone health assessments, and other diagnostic data to support clinical decision-making.
  • Develop individualized, evidence-based treatment plans, including pharmacologic and non-pharmacologic interventions, lifestyle counseling, and referrals when appropriate.
  • Prescribe and manage disease-modifying antirheumatic drugs, biologic agents, corticosteroids, and other therapies while monitoring efficacy, side effects, and medication safety.
  • Perform office-based procedures within scope of practice, such as joint injections, aspirations, and other rheumatology-related procedures as clinically indicated.
  • Educate patients and families regarding diagnoses, treatment options, medication risks and benefits, disease self-management, and long-term follow-up needs.
  • Collaborate with primary care providers, specialty physicians, nursing staff, rehabilitation professionals, and other team members to coordinate comprehensive care.
  • Provide timely consultation, follow-up, and clinical recommendations, including review of referral information and communication of care plans.
  • Maintain accurate, timely, and complete documentation in the electronic medical record in accordance with organizational, payer, and regulatory standards.
  • Support quality improvement, patient safety, utilization management, and evidence-based practice initiatives related to rheumatology care.
  • Participate in call coverage, peer collaboration, committee work, and organizational initiatives as assigned.


OTHER DUTIES:

  • Participate in provider meetings, peer review, continuing medical education, and ongoing professional development to maintain current knowledge and competency in rheumatology.
  • Assist with development of clinical protocols, referral workflows, and collaborative care practices that improve patient access, quality, and outcomes.
  • Assumes other duties and responsibilities as assigned.

Medical, dental, behavioral health and Express Care walk-in services as well as a full range of wellness programs make up the comprehensive primary care provided by Community Health at five locations in Vermont’s Rutland and southern Addison Counties. Express Care practices located in Rutland and Castleton provide walk-in care 7 days a week, 365 days a year from 8am until 8pm for treatment of minor injuries and illnesses. Our primary care practices are located in Rutland, Brandon, Castleton, Shoreham and West Pawlet, with dental practices in Rutland and Shoreham. All of our Community Health practices are accepting new patients and we accept most area insurance including Medicare and Medicaid. Inpatient hospital care for Community Health patients of all ages is provided by Rutland Regional Medical Center Hospital (RRMC).
