We are seeking a compassionate and motivated Case Manager to support veterans in obtaining and maintaining meaningful employment. This role places a strong emphasis on employment retention, working closely with veterans after job placement to ensure long-term success in the workplace. The ideal candidate is relationship-driven, resourceful, and committed to helping veterans achieve lasting self-sufficiency through stable employment.
Key Responsibilities
- Conduct comprehensive assessments to evaluate veterans’ employment readiness, skills, and career goals
- Develop individualized service plans focused on job placement and long-term employment retention
- Build and maintain relationships with local employers to identify job opportunities for veterans
- Provide ongoing support to employed veterans to promote job retention, including regular check-ins, coaching, and problem-solving around workplace challenges
- Coordinate with employers and community partners to support successful job placements and continued employment
- Connect clients to supportive services such as healthcare, behavioral health, and workforce development programs
- Monitor client progress and adjust service plans to support continued employment success
- Maintain accurate and timely case notes and documentation
- Advocate on behalf of clients to reduce barriers to employment retention
High school diploma with human services or social services experience. Bachelor's degree preferred.
Pay Range$40,000.00 - $44,000.00
