Big Bend Hospice is seeking a Referral Specialist to serve as the vital first connection between patients, families, healthcare providers, and our care teams.
In this full-time role, you will help coordinate referrals for Hospice, Home Health, and Palliative Care services, ensuring patients receive timely, compassionate support. Schedule: Full-time, M-F, 1:00pm - 10:00pm with rotating weekends and holidays.
What You'll DoAs a Referral Specialist, you will play a key role in creating a seamless admission experience for patients and referral partners by:
- Responding promptly and professionally to incoming referral calls.
- Gathering and verifying patient, insurance, and referral information.
- Coordinating admission visits with our clinical team.
- Scheduling appointments and ensuring timely communication with patients, families, physicians, and referral sources.
- Creating and maintaining accurate patient records within the electronic medical record (EMR) system.
- Collaborating with Admissions Nurses, Social Workers, Finance, and Patient Care teams to facilitate smooth admissions and reimbursement processes.
- Ordering equipment and supplies as requested to support patient care needs.
- Communicating referral updates to internal teams and community partners.
- Converting referrals to active admissions and maintaining accurate data throughout the process.
- Supporting organizational goals while delivering compassionate, customer-centered service.

- High School Diploma or GED required.
- Associate degree preferred.
- Minimum of two years of administrative or clerical experience required.
- Experience in a healthcare or medical office setting preferred.
- Knowledge of medical records and healthcare terminology preferred.
- Active Florida Licensed Practical Nurse (LPN) license is a plus.
- Ability to sit and work at a computer for extended periods.
- Ability to lift up to 15 pounds occasionally.
- Ability to access and navigate various departments within organizational facilities.
