Recovery Specialist

• Performs work associated with recovery support services, case management, treatment and rehabilitation of persons with drug and/or alcohol use.  Services and support will be delivered to patients in the primary care setting in conjunction with Medication Assisted Treatment (MAT).    
• In accordance with Patient Centered Medical Home philosophy, collaborates with team members including:  patient; primary care provider; behavioral health provider and others to deliver comprehensive care to patients of Family First Health through:
  • Patient Engagement and Empowerment
    • Participates in care coordination by utilizing case management tools, such as strength-based case management, motivational interviewing to facilitate patient retention in care and move patients toward independence and increased autonomy.
    • Assists participants in the management of their recovery by ensuring that the resources to address the participant’s needs are in place.
    • Provides drug and alcohol education, risk reduction support, and medication adherence guidance. 
    • Promotes participant retention, self-sufficiency in treatment and utilization of available support through educating participants about service availability.
    • Engages in brief relationship building and active listening with patients receiving treatment
    • Provides supportive (peer-based) coaching and encouragement to patients contemplating or actively seeking help with substance use treatment and recovery.
    • Supports patients in developing a self-directed recovery plan to meet their recovery goals
    • Will serve on-call duties based on on-call policy and departmental procedures.
  • Connection with other providers
    •  Identifies, develops and maintains positive linkages with agencies and social service organizations/services in the community and within FFH. 
    • Communicates with FFH interdisciplinary team, peers in other departments, managers, patients and other community agencies to ensure ongoing care for patients, maintain connectedness, and foster positive internal/external customer service. 
    • Facilitates warm handoffs between all available behavioral health and physical/dental health treatments. 
    • Creates and manages long-term individualized service plan by assessing patient’s target needs and reassess on timely basis, including making appropriate referrals for treatment and rehabilitation of drug and alcohol dependent patients. 
  • Documentation 
    •  Maintains timely and accurate documentation and tracking of all service provision, evaluation, and statistical data for the program; including detailed patient demographics, medical information, risk factors, and other information needed related to program funding. 
•  Understands and adheres to all state and federal drug & alcohol confidentiality laws and related statutes. 

• Attends and actively participates in both internal and external meetings as required. 
• Attends trainings and conferences as determined by supervisor. 
• Performs other duties as assigned and related to the position. 

• Must have a minimum of two (2) years uninterrupted recovery
• Must have or be eligible to pursue Certified Recovery Specialist (CRS) or Certified Family Recovery Specialist (CFRS) credential
• Must be actively engaged in activities that support a recovery oriented lifestyle
• High School Diploma or GED required. Previous education/experience with drug and alcohol programs preferred
• Excellent customer service skills with the ability to effectively establish appropriate boundaries 
• Strong written and verbal communication skills with the ability to support individuals of all backgrounds
• Ability to display critical thinking and exercise individual judgement and discretion in matters of significance 
• Ability to establish and maintain effective working relationships with patients, employees, and the public  
• Ability to be organized and work efficiently with strong attention to detail within a demanding work environment while appropriately prioritizing tasks
• Thorough knowledge of the Guiding Principles of Recovery.  
• Knowledge of MAT and ability to perform duties in the context of MAT. 
• Knowledge of computer software applications (Microsoft Office applications, health records, others) 
• Availability to work a 40 hour week between the hours of 7:00 am and 7:00 pm.
• Ability to set positive examples for team members through exceptional work performance including adherence to company policies and processes.
• Ability to work independently and within a multidisciplinary team
• Ability to work in, and respond appropriately to a demanding environment including crisis situations
• Ability to appropriately handle sensitive and confidential information
• Ability to be organized, proactive in follow-up and follow-through, and to manage multiple competing tasks
• Ability to adhere to all Family First Health’s policies, processes, and procedures
• Ability to articulate and advocate Family First Health’s mission and values.
• Ability to maintain required clearances and licensure.

PHYSICAL DEMANDS 

Mental/Physical Requirements: Must possess the physical and mental abilities to perform the tasks normally associated with a Recovery Specialist position such as:   

• Frequent and prolonged work at a computer and/or on the phone for extended periods of time with repetitive typing with arm and hand motion  
• Periodic travel between work sites with exposure to weather conditions  
• Requires correct vision and hearing to normal range 
• Occasional bending, stooping, crouching, or kneeling  
• Frequent standing, walking, turning, and reaching  
• Frequent contact with patients, clients, vendors, and visitors  
• Occasional carrying of office supplies and light equipment  
• Frequent competing demands, frequent changes, delays, or unexpected events  
• When teleworking or working remotely, requires working alone in a qualified work setting
