What You'll Do

• Coordinate and oversee resident care plans to ensure each resident receives individualized, high-quality care

• Monitor resident conditions, complete assessments, and communicate changes with physicians and the interdisciplinary team

• Provide leadership, support, and guidance to nursing staff while promoting a positive team environment

• Serve as a trusted resource for residents and their families by providing updates and addressing concerns

• Ensure accurate documentation and compliance with state, federal, and facility regulations

• Participate in quality assurance activities and help maintain excellence in resident care

What We're Looking For

• Current, active LPN license in the state of practice

• At least 2 years of nursing experience in skilled nursing, long-term care, or a similar healthcare setting preferred

• Strong knowledge of long-term care regulations and resident care planning

• Excellent communication, leadership, and organizational skills

• Ability to prioritize, problem solve, and work independently while collaborating with the interdisciplinary team

Skills Required
