I. JOB SUMMARY
This position has primary responsibility of one or more disease-specific and patient safety team by acting as facilitator (in a triad leadership model) to drive success to establish and/or manage maintenance of certification by The Joint Commission.
This position is responsible for abstraction of quality measures, performing interrater reliability on abstracted measures and evaluating any data outliers. Duties also include monitoring compliance and providing real-time coaching to providers and staff on data-driven improvements.
The position is responsible for continuous collaborative learning, including (but not limited to): identifying and supporting performance improvement projects and activities throughout the organization through education, planning and project oversight.
II. ESSENTIAL JOB RESPONSIBILITIES
- Implementation, evaluation and continuous improvement of regulatory and contracted value related clinical outcomes, such as, CMS quality reporting programs (Inpatient, Outpatient, and Clinics), CMS incentive/penalty programs, and third party payers.
- Perform an analysis of clinical outcomes to understand root cause when results are less than expected.
- Using Lean Six Sigma tools facilitate performance improvement projects.
- Actively engage in the culture of quality, safety, high reliability, patient and staff engagement, and performance excellence.
- Participate and/or support quality metrics as needed, such as, alignment of metrics across the continuum, abstraction and/or validation.
- Develop, implement and evaluate regulatory survey readiness activities.
- Participates in all educational opportunities identified as important to this position.
- Data interrater reliability and analysis of outliers (potential and actual).
- Analyze data and provide recommendations for performance improvement projects or actions.’
- Facilitate performance improvement projects
- Performs other duties as needed or assigned.

III. JOB QUALIFICATIONS
Required:
- Current applicable licensure (may consider working experience in clinical/healthcare setting in lieu of licensure)
- Minimum 3 years of working experience in a clinical/healthcare setting
- Bachelor’s Degree
Preferred:
- Master’s degree
- Clinical licensure (RN, Med Tech, etc.)
- Experience in process improvement
- Yellow Belt in Lean Six Sigma
- Detail oriented and organized approach to problem solving and PI initiatives.
- Advanced knowledge of computer spreadsheets and databases (including Microsoft Excel, Word and preferably Access).
- Basic knowledge of statistics and statistical methods of analysis.
- Good organizational skills; ability to prioritize, use good time management and adhere to deadlines as directed.
- Strong and collaborative team player; works well in a continuous learning environment.
