Position Purpose: Performs duties to assist in activities related to closing open care gaps for high-risk members.
- Interacts with high-risk members and/or providers to improve healthcare outcomes and close care gaps such as increased preventive care compliance
- Provides support for quality initiatives including outreach to members who are noncompliant in the designated outreach services
- Conducts seasonal call campaigns, in coordination with local market efforts
- Enters data assessments and authorizations into the system and maintain database as directed
- Maintains integrity of Personal Health Information
- Performs other duties as assigned
- Complies with all policies and standards
Education/Experience: High school diploma or equivalent. 2 years managed care setting, medical office or facility setting with demonstration of medical administration duties. Knowledge of medical terminology.
Pay Range: $18.66 - $31.73 per hour
