Quality Assurance Nurse - USRN NY

We are seeking a highly organized and detail-oriented Quality Nurse Analyst.

This position is remote and offers you the flexibility of working from home.

Essential Role and Responsibilities:

• Review clinical incidents.
• Follow standard operating procedures (SOP) to determine root cause, risk, and resolution for incident.
• Process interim medical orders as needed to notify physician of patient needs and changes in condition such as fall, wound, injury and post-hospitalization
• Perform follow-up calls for patients recently discharged from acute hospitalization and who are considered high risk for readmission
• Communicate as needed to client, caregiver, coordination teams, payers (case management / service coordination), field RNs, providers and key stakeholders to ensure client safety and continuity of care.
• Follow regulatory guidelines to manage clinical incidents to key deadlines.
• Share trends and insights noted in aggregate data to support continuous quality improvement.
• Collaborate regularly with assistant director of nursing services to review clinical cases and client needs.

Required Skills and Abilities:

• Detail oriented, highly organized, and self-motivated.
• Strong interpersonal skills – verbal and written communication
• Strong clinical judgment and care planning skills
• Collaborative with key stakeholders

Education and Experience:

• RN, NYS license
• Minimum of 2 years nursing experience
• Managed care experience preferred

Industry standards applicable to the state depending on work experience and level of expertise.
