JOB TITLE: Quality and Risk Coordinator

SHIFT: FTE 1.0 (80 hours bi-weekly) Full-Time, 1st shift

Summary/Objective

Under the general direction of the Director of Quality and Risk Management, coordinates and supports the organization-wide Patient Safety Program. This position administers the safety event reporting process, supports review and investigation of patient safety events, facilitates communication of safety information, and collaborates with leaders and staff to reduce preventable harm and strengthen a fair, learning-focused culture of safety.

Essential Functions

• Coordinate the organization-wide patient safety event reporting process, including review, triage, routing, follow-up, and closure of reported events and near misses.
• Assist the Director of Quality and Risk Management and appropriate leaders with investigation of patient safety events, including collection of records, development of event timelines, interviews, and identification of contributing factors.
• Coordinate and participate in Root Cause Analyses, Apparent Cause Analyses, and other structured reviews; document findings, recommendations, and assigned follow-up actions.
• Monitor event-reporting trends, recurring risks, near misses, and unsafe conditions; prepare dashboards, summaries, and reports for leaders, department managers, physicians, committees, and the governing body as assigned.
• Communicate patient safety information in a timely, clear, confidential, and nonjudgmental manner to appropriate stakeholders.
• Promote timely reporting of safety events and near misses and support a fair, nonpunitive, learning-focused approach to event reporting.
• Seek feedback from users regarding the functionality and value of the safety event reporting system and recommend improvements to support organizational learning.
• Serve as a patient safety resource to clinical and nonclinical departments and assist leaders in evaluating identified variations in care, risks, and opportunities for improvement.
• Collaborate with managers, physicians, staff, content experts, and executive sponsors to develop corrective action plans, assign measures, monitor implementation, evaluate effectiveness, and support sustained improvement.
• Assist in developing and revising policies, procedures, tools, and communication processes related to patient safety and event reporting.
• Develop, coordinate, and provide education on patient safety principles, event reporting, high-risk processes, and lessons learned from internal and external safety information.
• Review relevant external patient safety alerts and advisories and route information to appropriate leaders for evaluation and action.
• Support the Quality Oversight Committee and other committees or teams as assigned through agenda preparation, presentation materials, reports, and meeting minutes.
• Compile and disseminate quality and patient safety reports and identify gaps in measures or performance for follow-up with appropriate stakeholders.
• Assist with performance improvement teams, task forces, and patient safety initiatives under the direction of the Director of Quality and Risk Management.
• Serve as a resource for accreditation, CMS, and State licensing requirements related to patient safety, quality, and hospital policies and procedures.
• Assist with internal audits, tracer activities, regulatory or accreditation surveys, surveyor information requests, and corrective action plans as assigned.
• Promote evidence-based care and best practices throughout Cameron Health and associated entities.
• Assist the Infection Preventionist in completing Physical Environment rounds and reporting of findings to department leader and rounding software program.
• Serves as a backup to the Infection Preventionist as needed including:
  • monitor for infections
  • complete IDOH reports for communicable diseases
  •  Complete NHSN reporting
  • Assist with employee exposure investigations
  • Review positive culture reports

Qualifications

• Leadership skills
• Training/knowledge in regulatory requirements/standards
• Computer literate: must be able to demonstrate proficiency with Word, Excel, and PowerPoint
• Excellent verbal and written communication skills
• General knowledge of healthcare operations
• Previous quality improvement experience preferred

Experience

• 3 years clinical healthcare experience
• Experience in quality, patient safety, accreditation or regulatory work preferred.

Skills/Competencies

• Excellent written, oral, presentation, and interpersonal communication skills.
• Ability to work collaboratively with diverse teams and individuals in a professional and approachable manner.
• Knowledge of complex hospital systems, clinical processes, patient safety, and quality/performance improvement.
• Ability to organize and prioritize multiple responsibilities, meet deadlines, and work with minimal supervision.
• Ability to apply problem-solving and basic analytic methods to patient safety and quality information.
• High level of professionalism, discretion, and confidentiality.
• Ability to synthesize information from multiple sources and support development of policies, procedures, and improvement plans.
• Proficiency with Word, Excel, PowerPoint, Epic, event-reporting systems, tracer tools, and reporting tools.

Education Required

• High School Diploma
• Degree in Nursing, Health, Business, Sciences, Statistics or related field with a medical background and experience in the clinical setting preferred.

Certifications Preferred

• CPPS preferred.
• CPPS required within 3 years of hire
