POSITION SUMMARY:
The Psychiatric RN Case Manager demonstrates advanced professional nursing practice to improve access, coordination, safety, and outcomes for patients with psychiatric and behavioral health conditions within an integrated care setting. The Psychiatric RN Case Manager provides direct nursing interventions, psychiatric triage, medication and disease process education, care coordination, and collaboration with psychiatric providers, medical providers, behavioral health clinicians, community agencies, and support systems.
This role supports continuity of care through patient assessment, nursing follow-up, medication monitoring, risk assessment, crisis intervention, and interdisciplinary collaboration while promoting patient engagement, treatment adherence, and recovery-oriented care.
The Psychiatric RN Case Manager also serves as a clinical resource and educator to nursing peers and integrated care team members regarding behavioral health conditions, psychotropic medications, and management of high-risk psychiatric patients.

Specific Duties
- Conduct initial psychiatric nursing assessments for patients to assist with triage and placement with appropriate psychiatric providers and services.
- Conduct psychiatric nursing visits for medication reconciliation, medication education, symptom monitoring, side effect assessment, safety follow-up, hospital discharge follow-up
- Collaborate with psychiatric providers, primary care providers, therapists, recovery coaches, care coordinators, schools, DCF, DMH/DDS, detox facilities, hospitals, correctional facilities, community agencies
- Assist patients with referrals and placement into higher levels of care
- Monitor and coordinate laboratory testing for psychiatric medications
- Conduct or coordinate psychological assessment tools
- Conduct outreach and assist with scheduling for identified psychiatric care gaps
- Support same-day patient needs and resolution through nursing triage and coordination.
- Coordinate emergency psychiatric evaluations and crisis interventions as needed.
- Provide education and support to patients, families, support staff, interdisciplinary team members
- Assist in development of psychiatric nursing workflows, protocols, and educational materials
- Assist in development and maintenance of behavioral health resource materials and nursing reference tools.
Secondary Responsibilities:
- Assist with in-basket management and triage for psychiatric providers.
- Assist with behavioral health call center triage as needed.
- Complete follow-up appointments after missed visits or provider absences as assigned.
- Collect vitals, urine toxicology screens, oral swabs, and other specimens as needed.
- Support medication access barriers as assigned.
- Administer psychiatric long-acting injectable medications (LAIs) in collaboration with nursing and provider teams as assigned
- Support psychiatric providers with medication administration and health maintenance tasks as assigned.
- Participate in quality improvement initiatives related to behavioral health nursing workflows and patient outcomes.
- Participate in multidisciplinary team meetings and high-risk patient reviews.
- Quality Improvement & Data Collection
- Collect and assist with reporting data related to the Psychiatric RN Case Manager role including:
- Number of nursing visits
- Telephone encounters completed
- Medication reconciliation encounters
- Medication teaching encounters
- Same-day issue resolution
- Follow-up outreach attempts
- Hospital discharge follow-up completion
- Patient engagement outcomes
- Staff, provider, and patient satisfaction feedback
QUALIFICATIONS:
Required:
- Current RN license in the state of practice
- Psychiatric nursing experience (outpatient, inpatient, community-based, integrated care, substance use, or related settings)
- Experience working with patients with serious mental illness and/or substance use disorders
Preferred:
- Psychiatric Mental Health Nursing Certification (PMH-BC) or CARN certification
- Experience in integrated primary care/behavioral health settings
- Experience with crisis intervention and psychiatric triage
- Bilingual skills strongly preferred, especially Spanish, Khmer, Russian, Arabic, Haitian Creole, Portuguese
Skills Required
- Current Registered Nurse license in the state of practice
- Psychiatric nursing experience in outpatient, inpatient, community-based, integrated care, substance use, or related settings
