We are actively seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join a growing behavioral health program supporting residents throughout skilled nursing facilities, rehabilitation centers, long-term care communities, and post-acute settings across Green & Columbia County.

Skills Required
- Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active and unrestricted New York State Nurse Practitioner license
- DEA registration preferred or ability to obtain
- Comfortable practicing independently within long-term care environments
- Prior exposure to geriatric psychiatry, behavioral health, skilled nursing, rehabilitation, post-acute, or nursing home populations
- Experience managing dementia-related symptoms, chronic psychiatric illness, and psychotropic medication regimens
- Strong interpersonal, organizational, and communication skills
- Ability to work collaboratively within interdisciplinary healthcare settings
- Maintain timely, organized, and compliant clinical documentation
