About this role:
As a member of the agency’s management team, the Individual Supports Team Lead will provide leadership, supervision and daily management of all assigned tasks utilizing a team-based, multi-disciplinary approach in accordance with the Kennedy-Donovan Center (“KDC”) philosophy, mission and vision. The Individual Supports Team Lead reports to and has open and ongoing communication with the Individual Supports Director.
We are looking for people who have:
- Minimum of five years of recent professional health-care experience working with elders or adults.
- Excellent organizational, interpersonal and communication skills.
- Experience in providing home-based support required.
- Commitment to community-based support to individuals with developmental disabilities.
- Commitment to family-based services.
- Able to meet the required competency level in Microsoft Office Software and an Electronic Healthcare record.
The physical requirements for this role:
- Moderate travel required.
- Ability to meet with families and/or individuals within their homes.
- Ability to sit for extended time periods.
- Frequent phone use required.
- Ability to use a computer.
In this role, you’ll get to:
- Supervision of Individual Supports Case Managers to ensure program compliance and information sharing.
- In collaboration with Individual Supports Program Director maintain program compliance with all applicable state funding agencies and certification regulations and requirements as well as quality service standards
- Maintain program compliance with KDC’s mission, philosophy, standards, policies, and procedures.
- In collaboration with Individual Supports Program Director monitor all personnel activities, including hiring, training, supervision and evaluation of program employees
- In collaboration with the Individual Supports Program Director monitor completion and maintenance of documentation as required by KDC and all applicable funding agencies: Individual Support Plans, Incidents and Risk Management
- Manage and promote on-going collaborative relationships with funding sources and community service providers.
- In collaboration with the Individual Supports Program Director ensure regulatory compliance of all Individual Supports cases within the regulation timeline. This includes but is not limited to Case Notes, HCSIS documentation and iCentrix (electronic healthcare record) and consumer files.
- Manage on-going relationships with area directors of DDS and other funding sources to ensure positive interactions with KDC.
- Facilitate a team-based approach to program supports and services.
- Represent KDC with professionalism at agency functions, legislative and industry events.
- Provide emergency phone coverage as assigned.
- Other responsibilities as requested by the Individual Supports Program Director Case management of individuals as necessary. This consists of coordination of services by conducting home visits, documentation of all case management responsibilities, Individual Support Plans, HCSIS (home and community service information systems) and iCentrix (electronic healthcare record) as well as consumer files.
- In collaboration with the Program Director identify and facilitate staff training and development.
- Provide direct skill training, case management, supportive counseling and crisis management as directed and /or needed by the individual.

Kennedy-Donovan Center, Inc. (KDC) is a 501(c)(3) non-profit organization founded in 1969 on the principle that all individuals have the right to participate and thrive in their community. Kennedy-Donovan Center supports people with developmental delays, disabilities or family challenges to pursue their personal potential and success in the community.
We fulfill our mission through a number of programs, which we offer in several regions: Early Intervention (EI), Early Intervention Partnerships, Healthy Families, KDC School (a 240-day developmental day school), Day Habilitation Services, Family Support Services/ Respite Care, Intensive Foster Care, and Transitional Job Support, as well as various residential support programs: Shared Living, Adult Family Care, Supported and Supervised Living, and Individual Support.
Each year, our diverse and dedicated staff of 600+ employees and 300 respite home care providers and foster parents, serves over 10,000 children, adults and families with (or at risk for) developmental disabilities or delays, and their families. We meet the needs of disabled individuals and their families throughout their life – from pregnancy, infancy, and childhood, into adulthood and the senior years. We operate a range of programs, including many in-home services, from eight community offices and 10 residences, to families in 160 communities across eastern and south central Massachusetts.
