Job Summary
The Program Manager is responsible for the daily operations, leadership, and management of the CMS TEAM program and team, including oversight of the program execution to ensure goals, timelines, and budgets are met. CMS TEAM is a mandatory, episode-based, alternative payment model, in which Mass General Brigham will coordinate optimal care for Medicare patients undergoing one of five specified surgical procedures and assume responsibility for the cost and quality of care from surgery through the first 30 days after the patient leaves the hospital. This model aims to improve the patient experience preoperatively, through surgery and recovery. This will be done by supporting the coordination and transition of care between providers and promoting a successful recovery that can reduce unnecessary post-acute care utilization and readmissions.Essential Functions
- Administrative leader partner to clinical leader forming an effective, collaborative, equal dyad as complementary leaders of this program
- Accountable for the administrative operations of the program and main point of contact for the operations of the program
- Build and implement CMS TEAM program across MGB hospitals aligned with the organization's overall goals and objectives and manage effectively to deadlines and milestones
- Lead the design, development, and execution of 5 surgical episodes in the CMS TEAM program model across MGB hospitals
- Set program targets and goals and report out on gap to goal and remediation steps for improvement
- Effectively build, lead, and motivate teams
- Prepare and maintain the budget for assigned funds
- Track performance of the program to KPIs and targets and engage with hospitals and system leaders for performance and process improvement opportunities
- Communicate effectively and transparently to senior leaders including but not limited to Population Health Services Organization, surgical chiefs and leaders, care continuum and rehab, hospital, finance, healthcare at home, quality, etc.
- Collaborate effectively across departments and disciplines for problem solving, process improvement, alignment, and achievement of shared organizational goals, etc.
- Manage resources effectively to ensure the successful execution of programs.
- Build, lead, and mentor a high-performing team comprised of both clinical and non-clinical staff
Qualifications
Education
- Bachelor's Degree Related Field of Study or equivalent years of experience required
- Master's Degree Related Field of Study or RN/PT licensure or equivalent years of experience preferred
Experience
- 3 to 5 years of supervising and effectively leading staff and cross-functional teams experience required
- 3 to 5 years of complex program operations management experience required in a large health system preferred
- Thrive in high paced environment in building and launching new program
- Excited and motivated by the program goals of positively impacting patient care and making care less fragmented
- Empathetic collaborator with a genuine interest in improving quality of care for geriatric surgical patients
- Undaunted by roadblocks and possesses optimistic problem-solving skills

- Demonstrated experience in leading and managing cross-functional teams that are both clinical and non-clinical
- Possess both strategic skills and detailed operational experience to lead workstreams and staff teams, make decisions or tee up SBARs to inform leadership decision-making tactical workstreams
- Excellent strategic thinking and problem-solving skills.

- Adept at leading evaluation to assess impact of program and leverage data for process improvement
- Focus on process improvement to optimize clinical and non-clinical workflows and scale program effectively across the system Conduct and lead data-driven performance reporting with key constituents across the network
- Strong business acumen, data driven, and accountable for setting program targets and routinely reporting on performance KPIs
- Excellent communication and presentation skills.
