Primary Work Location:  

Natalie’s Place is one of several Cardea Health transitional housing programs for medically complex and behaviorally vulnerable unhoused individuals, located in Fremont. Cardea Health’s transitional housing programs collaborate with a wide range of referring partners across Alameda County to support individuals with complex health and social needs. Natalie’s Place provides comprehensive supportive services, including onsite nursing and caregiving support, transportation to medical appointments, nutritious meals, housing navigation and placement assistance, and additional wraparound services designed to promote stability, recovery, and long-term housing success.


Job Summary:  

The Program Manager of Transitional Housing will supervise and direct personnel and program activities for Cardea Health Transitional Housing Programs, including Natalie’s Place, a recuperative care program serving people who have experienced homelessness. The Program Manager is accountable for assisting with managing facilities, clinical personnel, housekeeping and caregiving functions, food service, transportation, and housing navigation. In addition, the Program Manager will work with the nursing team to manage the flow of clients through the program, from admission to discharge.

About the role:

Job Duties: 

  • Assist the Program Director of Transitional Housing and Director of Nursing for Transitional Housing with leading a team of dedicated clinical and non-clinical professionals   

  • Under the direction of the Program Director, oversee facilities and operations at a site of units   

Support the Program Director in manage personnel and operations related to:   

  • Clinical staff  

  • Case management  

  • Housekeeping   

  • Caregiving   

  • Food service   

  • Transportation   

  • Housing navigation   

  • Troubleshoot issues as indicated   

  • Escalate challenges when appropriate   

  • Manage client experience in the program from referral to discharge   

  • Assist with data reporting as required for assessment of program performance   

  • Maximize program efficacy in securing service connections and permanent housing for clients   

  • Ensure that clients move through program in accordance with program parameters  

  • Fill all beds promptly 


Basic Qualifications:

  • Excellent communication skills, both written and verbal, with the ability to interact professionally with diverse team members and clients   
  • Strong leadership skills, including team building   

  • Excellent organizational skills and attention to detail  

  • Excellent critical thinking and creative problem-solving skills 

  • Excellent time management skills with a proven ability to meet deadlines  

  • Ability to function well in a high-paced and at times stressful environment  

  • Display resilience and resourcefulness in tackling tasks and finding solutions  

  • Must provide proof of COVID vaccination or willing to be vaccinated  

  • Have a passion for serving marginalized populations  

  • Proficiency in Microsoft Office Suite is required  

  • Discretion and confidentiality in handling sensitive information  

  • Professionalism, integrity, and a proactive approach to problem-solving  


Minimum Required Skills & Qualifications: 

  • Bachelor’s degree or equivalent professional experience   

  • At least two years of related experience required  

  • Proven experience as a team leader and operations professional   

  • Experience working with people experiencing homelessness preferred  

  • Experience working in healthcare preferred   


Physical Requirements:

  • Occasional sitting or stationary work 

  • Frequent standing, walking, kneeling, stooping, and ascending stairs 

  • Occasional lifting and carrying up to 20 lbs 

  • Frequent pushing and pulling 

  • Frequent reaching high, low, and level 


Work Environment:   

  • In-person onsite at a supportive housing facility. Constant work around and/or with people OR Interaction with patients, clients, and external customers
