POSITION SUMMARY:
Community Health is seeking a Nurse Practitioner or Physician Assistant to work at our Rutland, Castleton and Brandon clinics. Candidates are expected to be knowledgeable and skilled in the delivery of primary care. Responsibilities include, but are not limited to:
• Obtains complete medical history and physical examination on patients.
• Reviews medical records.
• Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.
• Establishes presumptive diagnosis.
• Orders appropriate laboratory and diagnostic procedures.
• Develops a therapeutic plan for patient.
• Sutures wounds.
• Prepares written prescription orders for drugs and controlled substances.
• Instructs patients and family regarding medications and treatment instructions.
• Provides recommendations regarding health and illness prevention.
• Recommend community resource to meet patient/family needs.
• Performs independent evaluations and treatment procedures in emergency situations.
• Provides monitoring and continuity of care between physician visits.

Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.
KNOWLEDGE, EDUCATION AND SKILL REQUIREMENTS:
- 3-5 years Primary Care Practice Experience.
- Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.
- Licensed for Nurse Practitioner or Physician Assistant license in the state of Vermont.
- Strong inpatient skill set.
- Quality-focused team player.
- Excellent organizational, time management and problem-solving skills.
- Ability to function effectively as part of a multi-disciplinary team.
- Effective oral and written communication skills.
- Commitment to providing exceptional patient care.

What We Do
Medical, dental, behavioral health and Express Care walk-in services as well as a full range of wellness programs make up the comprehensive primary care provided by Community Health at five locations in Vermont’s Rutland and southern Addison Counties. Community Health providers offer primary, behavioral and preventative health care for people of all ages including immunizations, annual checkups for work, school and sports, chronic care management, minor office procedures, workers comp injuries, women’s health care and well-child care. X-ray and laboratory services are available at some of our locations, and we operate a pharmacy at our Brandon location. Community Health Pediatrics, located in Rutland, offers a complete range of specialized care for children and young families. Express Care practices located in Rutland and Castleton provide walk-in care 7 days a week, 365 days a year from 8am until 8pm for treatment of minor injuries and illnesses. Our primary care practices are located in Rutland, Brandon, Castleton, Shoreham and West Pawlet, with dental practices in Rutland and Shoreham. All of our Community Health practices are accepting new patients and we accept most area insurance including Medicare and Medicaid. Inpatient hospital care for Community Health patients of all ages is provided by Rutland Regional Medical Center Hospital (RRMC).
