- Deliver longitudinal primary care to Clover members in their homes.

- Located in or near Atlanta, Georgia.
- Able to travel locally for in-home visits.
- At least 1 year of PA experience in a clinical setting.
- Active DEA registration.
- Current Basic Life Support certification.
- Comfortable managing chronic illness and medically complex patients.
- Confident working with autonomy while collaborating with physicians and care teams.
- Compassionate, grounded, and able to build trust quickly with patients and families.
- Organized, responsive, and comfortable using technology to support care delivery.
- Home care, house call, geriatrics, complex care, or value-based care experience is a plus.
