Essential Job Functions
- Greet and communicate with patients in trauma-informed and respectful manner.
- Providing ready-to-pick-up medications to patients, including accessing the pharmacy operating system to determine if prescription is ready for pick up or delivery
- Schedule follow up appointments for patients, as communicated from various integrated health team members
- Assisting with packaging and processing medication orders for site delivery
- Obtaining documentations and filing paperwork as directed by pharmacists
- Record all clinical services in a timely and complete fashion in the EHR and RXQ (pharmacy operating system)
- Maintain up to date records in EHR & RXQ
- Record telephone and other clinical communications as needed in the EHR & RXQ.
- Manage tasks, PAQ and scanning as needed to have a complete and current chart.
- Maintain accurate information regarding client’s insurance status and complete CCARs/DACODs and other clinical authorization documentation as needed.
- Performs other duties as assigned.
Qualifications Summary
- High school or equivalent experience
- 6+ months of customer service experience
- Consistently superb customer service skills; excellent interpersonal/assertive communications skills demonstrating a high degree of emotional intelligence.
- Knowledge of homeless issues and demonstrated sensitivity to and a desire to work with underserved populations.

What We Do
The Colorado Coalition for the Homeless is a 501(c)(3) non-profit organization dedicated to addressing the causes and consequences of homelessness in Colorado. They provide a comprehensive approach that includes housing, integrated healthcare, substance use treatment, employment services, and childcare. Their mission is to improve the health, well-being, and stability of individuals and families experiencing homelessness through compassionate, patient-centered care and advocacy for justice and long-term housing stability.
