The Pharmacy Care Coordinator is responsible for providing clinical and technical support under the supervision of a registered pharmacist to coordinate ongoing care and treatment plans for Optum patients. Their duties involve interaction with customers, members, patients, providers, internal departments and/or external third parties.
Work Hours: 8:00am-5:00pm PST

Primary Responsibilities:
- Consistently exhibits behavior and communication skills that demonstrate Optum's commitment to superior customer service, including quality, care and concern every internal and external customer
- Coordinate patient care as directed by clinicians, company standards, and policies.
- Works as a member of the Pharmacy Care Management team by assisting clinical pharmacists to resolve medication related issues or problems
- Proper, concise, and complete documentation of pertinent patient information in the electronic medical record
- Assists with data management and recording results for month-end reports
- Assists in Pharmacy Services wellness programs (i.e. transition of care medication reconciliation and disease management clinics) by:
- Retrieving pertinent patient information for new patient referrals or recent hospital/SNF discharge
- Contacting patients post hospitalization to perform medication review and information gathering
- Performing patient "Missed Appointment / No Show" procedures
- Answering incoming phone calls and directing patient messages
- Collaborating with other members of the health care team
- Uses, protects, and discloses Optum patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
- Updating job knowledge by participating in educational opportunities
- Participating in process improvement initiatives, site staff meetings and departmental meetings
- Always Adhering to Universal Precautions and safety policies and procedures
- Other duties as assigned

Required Qualifications:
- High School Diploma/GED
- 1+ years of experience as a pharmacy technician
- 1+ years of experience in customer service
- 1+ years of experience with medical and pharmacy terminology
- 1+ years of experience with medication therapeutic categories
- 1+ years of experience with identifying generic name for brand name drugs and vice versa
- 1+ years of experience with an understanding of indication for medication
- Basic level of proficiency in Windows and MS applications (Word, Excel)
Preferred Qualifications:
- National Pharmacy Technician Certification
- Possess and maintain an active pharmacy technician license in the state of Washington and/or CA
- 3+ years of experience working as a pharmacy technician
- 1+ years of experience in hospital, ambulatory care or other healthcare setting
- Experience in managed care setting

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $17.98 to $32.12 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
