Educational Requirements:
- Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction
- Completion of a PGY1 or PGY2 ambulatory care residency or comparable experience in ambulatory clinical pharmacy
Preferred Qualifications:
- Completion of an ACPE accredited Medication Therapy Management training program
- Recent clinical practice, reflective of scope of this role within the previous 12 months
- Applicants must be board certified or become board certified within one year of eligibility of an acceptable board specialty certification including: BCACP, BCPS, or BCGP
- Active license to practice pharmacy within the states of MN and WI
- Ambulatory - Immunization Delivery Certification completed within 1 year from hire date
- Ambulatory - BLS certified within 90 days from hire date
