Pay Range
$57.85 - $86.80Major Responsibilities:
- Determine suitability, assess, monitor and manage care of patients receiving home infusion therapy as needed. Collects & interprets information, creates patient medical record and care plan, monitors & documents patient progress.
- Communicates patient status, progress updates, developments in care plan, treatment recommendations to all professionals involved in managing patients care plan as well as the patient.
- Prepares and dispenses medications using appropriate aseptic technique. Follows appropriate safety and quality measures. Follows all state/federal regulations regarding medication dispensing and provision of pharmaceutical care
- Supervises and utilizes pharmacy support staff effectively. Checks accuracy of the work completed by support staff in accordance with regulatory requirements.
- Manages the use, problem solving & maintenance of medication administration equipment.
- Provides drug and homecare resource information to patients, nurses, referral sources and others as requested and as needed.
- Complies with all regulatory and accreditation laws or standards related to the practice of pharmacy, as well as all AdvocateAurora Health policies and procedures.
- Participates in department and organization improvement initiatives

SUPERVISORY/MANAGEMENT RESPONSIBILITIES
Though not responsible from an HR perspective, the incumbent does assist the department pharmacy manager with the daily oversight of support personnel.
MINIMUM EDUCATION AND EXPERIENCE REQUIRED
License/Registration/Certification: Pharmacy licensure/registration per state requirements. Board certification preferred. Willingness to obtain licensure in multiple states preferred.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES
- Ability to operate all equipment for producing sterile product admixtures
- Ability to operate pharmacy data processing system
- Exceptional verbal communication skills
- Strong attention to detail
- Strong organization skills
- Ability to provide 24/7/365 support to patients as part of an on-call rotation
- Ability to occasionally lift up to 35 lbs.
- Ability to stand for long periods when in clean room environment
- Ability to comply with all sterile compounding garbing requirements
