We are seeking a Performance Improvement Specialist to join our team. 

Hourly Pay Range: $18.00 - $20.66

This hybrid remote role will ensure quality services are provided to the children and families served throughout Cornerstones of Care. This position assesses and monitors programs through the agency’s Performance and Quality Improvement (PQI) framework, including structured review, analysis, and improvement processes to ensure the outcomes and process requirements of the contracts, accreditation, and licensure bodies are met while ensuring documentation practices and electronic workflows accurately capture meaningful outcome data. The Specialist will work with program staff of all levels to support, develop, and improve the quality of services being delivered to clients served including evaluating how documentation, data entry processes, and reporting structures impact measurable program outcomes.

WHAT YOU WILL DO:

  • Compile, analyze, and share results and trends from monthly and quarterly reports for each program to report outputs and outcomes on performance measures and assist with internal programmatic benchmarking opportunities including identifying emerging risks, gaps in performance, and opportunities for data-informed improvement.
  • In collaboration with program leaders, evaluate and synthesize data trends to develop and implement improvement initiatives using structured improvement methodologies and clearly defined outcome measures.
  • Facilitate or co-facilitate EQUIP (PDSA) projects to test, measure, and refine improvement strategies based on identified trends.
  • Work collaboratively with the Cornerstones electronic health record (EHR) partner to develop reports and support utilization of full implementation for data integrity including reviewing proposed system enhancements for workflow efficiency, documentation clarity, and downstream reporting impact prior to implementation.
  • Identify, investigate, and resolve any discrepancies between agency and stakeholder outcome data.
  • Support ad-hoc analysis as required for grant reporting.
  • Meet regularly with program leaders/operations to identify improvement opportunities.
  • Other duties as assigned.

WHAT YOU WILL BRING:

Our ideal candidate will have 2-3 years of relevant experience and the following:

  • Bachelor’s degree, required.
  • 2+ years of excel experience, required.
  • At least 21 years of age and pass background check, physical, and drug screening.
  • A valid driver’s license in the state you reside in, proof of current vehicle insurance, and reliable transportation.
