As a Performance Improvement Specialist for crisis you will oversee follow up care for those transitioning from crisis, compliance of documentation with external funding sources, and assist with agency performance improvement reports.


To meet the increasing needs of our communities it is imperative that all team members understand and successfully reach and maintain the expected performance standard levels. In order to achieve total and sustainable success, each person must do their part. Lighthouse will do its part in providing any and all necessary tools and training required to help team members be successful.


DEFINITION:


Under immediate to general supervision, monitors follow up requirements by ODMHSAS for individuals in crisis and provides follow up calls for continuity of care.  Completes chart reviews for compliance with ODMHSAS and OHCA standards, completes quarterly client satisfaction surveys, and contributes to quarterly performance improvement reports.  Monitors CDC’s for errors and assists crisis leaders with needed corrections. This position is supervised by the Manager of Continuous Quality Improvement (CQI).


Qualifications

EDUCATION AND EXPERIENCE:


  • Must have a bachelor’s degree in a mental health related field from an accredited university and two years of performance improvement and/or clinical documentation experience or a master’s degree in a mental health field with one year of performance improvement and/or clinical documentation experience.  May also have experience in providing direct services in a mental health related field.
  • Must meet ODMHSAS requirements for certification as a Case Manager II within 3 months of employment and maintain certification annually.

KNOWLEDGE AND SKILLS:


  • Knowledge of the principles and practices of social work and/or psychology; and of current social and economic problems.
  • Thorough skill in exercising good judgment in analyzing situations and making decisions; in organizing and presenting facts and opinions clearly and concisely, both orally and in writing; in establishing and maintaining effective working relationships with other Lighthouse personnel and governmental representatives; and in organizing and conducting projects simultaneously.
  • An expectation of core competencies in relation to individuals with co-occurring disorders, trauma informed care and collaborative care.
  • Knowledge of case management procedures and integrated care.
  • Knowledge of inpatient, crisis stabilization and outpatient services for adults and children.
  • Must be a self-starter and able to work independently.
  • Must develop working relationships with clinical staff and supervisors.
  • Must possess the ability to train staff and assess for comprehension of information
  • Must be very detail oriented.

JOB DUTIES AND RESPONSIBILITIES:


This position is responsible and held accountable for the following duties:


  • Follows guidelines and ensures compliance of clinical records for Title XIX; Medicaid, Oklahoma Department of Mental Health and Substance Abuse Services (ODMHSAS), and other funding sources or regulatory agencies.
  • Reviews clinical records for funding source and accreditation standards and instructs staff on chart correction and compliance on a regular basis.
  • Monitors OMDHSAS requirements for follow-up care for individuals experiencing a mental health crisis.  Provides case management to individuals after a crisis service or crisis unit stay and works to ensure the individual receives appropriate transition to after-care services.  Works with both crisis and outpatient teams for transitional care.
  • Ensures that ETPS measures for follow-up care are being monitored for positive results.
  • Monitors CDC’s and documentation required for billing crisis services and works with supervisory team members on needed corrections.  Works with the Manager of Revenue Cycle when billing issues are identified. 
  • Ensures completion of NODOS process for individuals entering crisis services as needed.
  • Travels to Lighthouse sites (or provides via Zoom) to ensure agency wide compliance and to provide staff training.
  • Assists in data collection by completing quarterly reports for consumer satisfaction (if assigned).
  • Monitors service verification during chart reviews.
  • May provide assistance in updating and maintaining accurate workflows for outpatient and/or crisis services and assist with staff training on workflows.
  • Assists in recording data and preparing compliance reports under the direction of the Manager of CQI.
  • Assists in preparing for certification audits.
  • Works collaboratively and communicates effectively with direct supervisor, site supervisors, and clinical staff.
  • Regular and predictable attendance is an essential job requirement.
  • Reviews and understands service code definitions and has the ability to relate the service code definition to the chart documentation.
  • Employee must be willing and able to perform all job related travel normally associated with this position.
  • Must meet “satisfactory” expectations at any scheduled job performance evaluation, coaching and/or Performance Improvement Plans.
  • Must complete Lighthouse required training and resulting follow up and consultation as required at hire, annually and as directed by Supervisor.
  • Performs other reasonably related duties as assigned by the immediate supervisor or other management as requested.

In addition, must possess skill in working cooperatively and effectively with clients, staff, management and other professional and community groups; in exercising mature judgment in dealing with people; in presenting ideas clearly and accurately; in reading and comprehending the English language; and in communicating effectively, both orally and in writing.
