Thresholds is seeking a compassionate and dedicated Peer Support Specialist to join our Wellness West Care Coordination Program. In this role, you will leverage your lived experience to support individuals navigating mental health and physical health challenges in person on the West Side of Chicago. Wellness West services individuals who suffer disproportionately from higher rates of chronic disease, acute ailments and mental illness. You will serve as a member advocate, helping individuals regain control over their lives and recovery processes while modeling resilience and coping skills.

To succeed in this role, you need:
- Lived and living experience with personal mental health recovery
- Passion for mental health advocacy
- Effective communication and relationship-building skills
- To be at least 21 years old
- A valid driver’s license, current car insurance, and daily access to a car
- Willingness to provide transportation to clients
Education and experience:
- High school diploma or GED certificate required
- Active CRSS credential preferred

- Competitive pay - Base Rate: $21.75 - $24.42 hourly / $45,240 - $50,800 annually
- Commensurate with education and experience
- Additional increase with current CRSS
