As the Peer Navigator, you will play a pivotal role in individuals' lives, making a significant impact through:  

  • Make initial contact with eligible participants within 3 hours of receiving referral
  • Facilitate initial intake with referrals
  • Deliver outreach and engagement services in accordance with the service plan
  • According to program guidelines, consult with case management staff, Health HUB staff and/or participant’s providers on progress of service/care plan services
  • Conduct prevention related harm reduction/risk reduction interventions with participants 
  • Provide linkage and navigation to needed services for family members and other identified support systems
  • Maintain case records and documentation in accordance with agency and OASAS standards

  

Peer Navigator Minimum Salary: $ 20.51

  

Peer Navigator  Maximum Salary: $ 20.51

  

Peer Navigator Schedule:  Mon - Fri (37.5 hours/week)
 
 

  

Qualifications:   

  • High School Diploma or GED
  • Experience working with a diverse population
  • Valid driver’s license, proof of current car insurance and registration and access to a reliable, working vehicle
