The primary location for this role is remote, with quarterly travel. Expected schedule is Monday - Friday, 8:00am-5:00pm PST. Bilingual in Spanish highly preferred for this role. Washington or California state license required.
What You’ll Do
The Clinical Pharmacist, as part of the interdisciplinary care team, provides medication management services for pediatric patients with chronic medical and behavioral health conditions focused on optimizing medication therapy, disease outcomes, and value of care. The Clinical Pharmacist is responsible for the day-to-day operations medication management services within the designated region.
In this role you will:
- Provide medication management services inclusive of medication access, medication education, medication adherence, comprehensive/directed medication management, and quality metric support for the designated region in alignment with the programs as designed by the medication management center of excellence.
- Assist with medication access management services between the patient, pharmacy, insurance, and medical provider through oversite and support of the region Medication Support Navigator.
- Provide accurate, thorough medication education, medication adherence assessment, and comprehensive/directed medication management through direct interactions with patients/caregivers, internal and external care team members, and works independently under individualized scope of clinical practice (where applicable).
- Document encounters in the patient electronic medical record, including closed-loop communication with patient care team.
- Promote the practice of evidence-based, cost-effective medication management by keeping abreast of new drugs, devices, and medical literature.
- Document/report adverse drug events (ADEs), medication allergies and medication errors per procedure.
- Liaison between the designated region and the medication management center of excellence.
- Practice pharmacy in compliance with all governing regulatory authorities.
- Perform other duties as assigned.

Education & Training Requirements
- Graduate of an accredited college/university of pharmacy (Pharm.D) required
- Must hold an unrestricted, current pharmacist license with ability to obtain licensure by reciprocity in each state where telehealth services will be provided.
- Active license in Washington or California required
- Completion of ASHP accredited PGY-1 Pharmacy Residency, preferred settings include pediatric or ambulatory care, required
- Completion of an ASHP accredited PGY-2 Pediatric or Ambulatory Care Pharmacy Residency, preferred
- Board Certified Pediatric Pharmacy Specialist (BCPPS), Board Certified Pharmacotherapy Specialist (BCPS), OR Board-Certified Ambulatory Care Pharmacists (BCACP), preferred
- Applicants who are not board-certified will need to be eligible for board-certification in a preferred specialty area and will be expected to pursue and successfully achieve certification within an agreed upon period
- Bilingual English/Spanish preferred
Work Experience
- 2+ years of practice experience in pediatrics, post-PGY1 residency
- Pediatric or ambulatory care setting strongly preferred

This role offers a base salary range of $130,000 - $150,000 in addition to annual bonus incentive, competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). When determining compensation, we analyze and carefully consider several factors including job-related knowledge, skills and experience. These considerations may cause your compensation to vary.
