Job Description

Position Summary

The Outpatient Coordinator coordinates the Assisted Living and Residential Living therapy services along with any outpatient services needed.  Assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and support recovery from injury or illness.  May include additional coordination of team member to ensure that all services can be performed as needed for residents, and guests.  This role is grounded in building strong relationships with therapy staff, residents, and the broader care team - fostering a culture where every individual's unique strengths and perspectives are recognized and valued.

Essential Duties

  • Coordinates needed rehab services for residents, guests, and includes outpatient services.  
  • In partnership with the Director, ensures efficient and effective operations for physical therapy, occupational therapy, and speech departments.
  • Takes time to understand each resident's goals, preferences, and comfort level, fostering trust throughout the treatment process.
  • Restores residents’ function, alleviates pain, and prevents further limitations by planning and administering medically prescribed physical therapy.  
  • Builds strong, trust-based relationships with residents, families, and staff to achieve high satisfaction with healthcare delivery alongside financial goals.
  • Evaluates the use and fit of prosthetic and orthotic devices, observing resident progress and recommending adjustments and modifications.
  • Communicates resident progress with warmth and clarity to residents, caregivers and loved ones, facility staff, and other healthcare professionals involved in ongoing care.
  • Interacts with interdisciplinary team members, monitors compliance, manages insurance/billing issues, and coordinates clinical aspects of the rehab program.
  • Develops a transition plan for residents following completion of their Plan of Treatment, coordinating appropriate in-home care services.
  • Documents resident care services according to approved medical and facility documentation procedures.
  • May train, and collaborates with therapy and nursing staff to strengthen communication and teamwork, and to ensure a positive experience for facility residents.
  • Champions an inclusive, supportive workplace culture where diverse backgrounds, perspectives, and working styles are welcomed and celebrated among staff.

Qualifications

Basic Qualifications & Experience

  • Education: Graduate of an accredited therapy program.
  • Experience: 3–5 years of related experience, or an equivalent combination of education and experience.
  • Licenses/Certifications: Current state license in therapy discipline; CPR certification.
  • Excellent verbal & written communication, active listening, and social perceptiveness
  • Relationship building, compassionate leadership, and genuine appreciation for individuality and diverse life experiences
  • Critical thinking, sound judgment, and decision-making
  • Time management and service orientation
  • Promotes active learning and staff development
  • Collaborative teamwork

Working Conditions / Physical Requirements

  • Position requires medium-heavy physical work. Heavy lifting, pushing, or pulling objects up to 100 pounds of force occasionally and/or up more than 45 pounds of force repeatedly to move objects.
  • Is subject to frequent interruptions.
  • Interacts with residents, family members and visitors frequently.
