    We are currently looking for an: Director of Central Access - PEO to join our team of dedicated difference makers!

    SUMMARY:

    Responsible for building, creating, and maintaining central navigation pathways for connecting young people to supports and services at the point of entry into PEO. Develop, expand, and maintain resource/partner relationships to provide best coordination of services for young people. Formalize central access navigation model following best practices throughout target areas. Ensures that services and supports are focused on the seven target areas for young people transitioning into adulthood: Education, Employment, Housing, Finances, Transportation, Mental/Physical Health, and Permanency.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:
    • Coordinates efforts between Central Access Navigators, community service providers/partners and the young people to create pathways plans that contain the most appropriate supports and services available in a timely manner.
    • Manages and receives all referrals submitted to PEO, ensure referrals are assigned to a Central Access Navigator efficiently.
    • Develop and maintains relationship with coaching service providers (CSI, Central Plains, and OHB), manage openings and submit referrals for coaching when needed.
    • Develop, conduct, and implement Central Access consultations, ensure the Common Referral Form is completed and entered in the database, and refer for any additional independent living evaluations deemed necessary.
    • Create and maintain documentation, marketing materials and forms that are essential to the growth and effectiveness of Central Navigation.
    • Develop written protocols and procedures for central navigation and ensure proper implementation
    • Maintain and expand the PEO provider referral network by building community partnerships and expanding pathways for young people to provide the best coordination of services.
    • Train central access navigators, necessary staff and participating young adults to provide group and one-on-one orientations to new PEO young people. Conducts regular/periodic PEO orientation sessions.
    • Facilitate and coordinate community collaboration efforts for direct services providers who serve young people in Douglas and Sarpy County.
    • Work to identify policy changes, supports, and resources that can assist PEO young people in the seven target areas for young people transitioning to adulthood (listed in Summary above).
    • Attend Project Everlast, partner programs, and community meetings as needed.
    • Recruit, interview, and supervisor social work practicum students. Attend any required/necessary meetings and recruitment fairs. Completing and maintain all required documentation and credentials.
    • Build/expand and maintain criminal justice reentry pathways for PEO Central Navigation. Attend and participate in juvenile justice community committees.
    • Work with correctional centers, residential treatment centers and community providers and young people to build and maintain pathways for young people transitioning back into the community successfully.
    • Assist with the development, outreach and coordination of housing, transportation, and maternity programs
    SUPERVISORY RESPONSIBILITIES: This position does not have direct supervisory responsibilities of PEO employees. However, it does include the management of practicum students.

    WORK DIRECTION RECEIVED: Works with minimal direction.

    SCOPE/NATURE OF DISCRETION: Delegated discretion; authority to initiate and carry out actions that are deemed essential.

    QUALIFICATIONS:

    EDUCATION AND EXPERIENCE:

    Required:
    • Master's Degree in social work
    • Minimum of 2 years' experience working in the child welfare system or ability to demonstrate extensive knowledge relating to supports and services available for young people aging out of the foster care system.
    Knowledge:
    • Working knowledge of Microsoft office suite including Word, Excel, PowerPoint and Outlook
    • Office equipment and computer software
    Abilities:
    • Excellent organization and time management skills.
    • Excellent writing and editing skills.
    • Strong oral and interpersonal communication skills.
    • Establish and maintain effective working relationships
    • Ability to work collaboratively with individuals from a variety of backgrounds and experiences.

    LANGUAGE SKILLS:
    • Ability to read, analyze and interpret documents such as grant solicitations or governmental regulations.
    • Ability to write grant proposals and progress reports as well as routine reports and correspondence.
    • Ability to respond to common inquiries about the organization from the public and/or business community.
    • Ability to effectively present information to public groups.

    MATHEMATICAL SKILLS:
    • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
    • Ability to compute, rate, ratio, and percent and to draw and interpret bar graphs.
    • Ability to read and interpret demographic and statistical research data.

    REASONING ABILITY:
    • Ability to solve practical problems and deal with a variety of variables in unique situations.
    • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
    • Ability to define problems, collect data, establish facts, and draw valid conclusions.
    • Ability to interpret statistical research findings (such as Census data) and to interpret mathematical data accurately in written communications.
    • Ability to plan, schedule, and manage multiple activities at once

    PHYSICAL DEMANDS:
    • While performing the duties of this job, the employee is regularly required to sit and talk or hear.
    • The employee frequently is required to use hands
    • The employee is occasionally required to stand, walk, and reach with hands and arms.
    WORKING CONDITIONS:
    • Flexible schedule including possible evenings and weekends,
    • Willingness to travel, including overnight stays.
