What You’ll Do
As a Wellness Nurse, you’ll serve as a coordinator, educator, and advocate, supporting residents in staying healthy and independent.
Key responsibilities include:
Wellness & Prevention
- Lead wellness programming and health education
- Promote healthy aging and chronic condition management
- Conduct assessments to support overall well-being
Supporting Independence
- Connect residents with resources and services that help them remain in their apartments
- Recommend safety adaptations and assistive supports
Care Coordination
- Coordinate weekly on-site physician clinic visits
- Assist with lab orders and physician communication
- Maintain accurate resident records, forms, and emergency contact information
Collaboration & Transitions
- Partner with the Healthcare Navigator to support residents and families
- Guide transitions to higher levels of care when needed
Resident & Family Support
- Provide education, guidance, and emotional support
- Respond to concerns and help navigate the aging journey
Qualifications
- Active RN or LPN license (Illinois)
- 2+ years experience in geriatrics, community health, or case management preferred
- Current CPR/BLS certification
- Strong communication and coordination skills
- Passion for senior wellness and independence
Compensation & Benefits
- $42–$45/hour (based on experience)
- Up to 21 days PTO
