POSITION SUMMARY:

Our Community Nutritionists (RDs) work as part of a multidisciplinary team, providing nutrition intervention and counseling to a diverse patient population. The nutritionist may also plan, organize oversee and/or facilitate nutrition related events, activities, and workshops.

JOB FUNCTION:  

Responsibilities include, but are not limited to

  • Evaluates and monitors patients’ nutritional status.
  • Conducts nutrition education and counseling sessions.
  • Organizes and develops nutritional care plans and individualized therapeutic diets as needed.
  • Completes necessary reports and forms and documents patient services in the appropriate medical record.
  • Facilitates related services for health center clients as appropriate with respect to their confidentiality and privacy.
  • Organizes and distributes nutrition education materials.
  • Identifies resources for nutrition education materials and services as needed.
  • Conferences with the Director of Nutrition as necessary to seek consultation and share information that is pertinent to patient care.
  • Plans, conducts and participates in nutrition education presentations, workshops and health fairs for the agency.
  • Attends conferences and participates in trainings/informal sessions as requested by Director of Nutrition.
  • Participates in clinic and central office team meetings as designated.
  • Directs prenatal clients to local WIC offices and aids with their admission into the program.
  • Performs other related duties, as assigned.


WHAT WE LOOK FOR:

  • All of our Nutritionists must be Registered Dietitians (RD).   
  • New York State licensure preferred.
  • Minimum of B.S. in Nutrition or Health Education with experience.
  • Bilingual Spanish is a plus.
  • This role will require travel between Jamaica Queens and East NY Brooklyn.
