Position Summary: Participates in the assessment, planning, implementation, and evaluation of clinical transformation activities for an assigned service or population. In collaboration with the clinical team and Medical Staff, this position is responsible for the assessment, development, implementation, and evaluation of clinical outcomes and processes supporting patient care.
Essential Duties and Responsibilities:
Nursing Quality Improvement
- Collaborates in the development, implementation, and evaluation of clinical services and quality initiatives.
- Supports key strategies to drive quality improvement to meet strategic and clinical excellence objectives
- Facilitates interdisciplinary, collaborative activities to support the development, deployment and monitoring of safety and quality improvement initiatives.
- Gathers, analyzes and reports data related to performance and achievement of project objectives and timelines. Tracks issues and works to improve processes, efficiency and compliance of best practice standards including electronic documentation standards.
- In collaboration with key stakeholders, supports and facilitates the standardization of best practices to promote high reliability.
Data collection
- Accurate and timely quality data submission to external databases for benchmarking reporting
- Generate performance improvement reports from quality databases, including monthly reports for distribution/analysis of findings as well as ad hoc reports
- Summarize, analyze, and present key data findings to organizational leaders and key stake holders to drive improvement in quality priorities and nurse-sensitive quality indicator performance.
Policies
- Develop, implement, evaluate and revise nursing care policies, procedures, and guidelines aligned with best practices and quality initiatives.
Other
- Serves on specialty and/or unit committees, councils, and teams, as well as professional organizations.
- Facilitates compliance with Joint Commission standards and regulations of other appropriate regulatory agencies.
- Maintain current knowledge of industry standards by proactively assessing changes in healthcare regulation, accreditation standards and operational best practices through available resources and professional development opportunities to serve as educational resource to the organization
- Handles sensitive, confidential information responsibly
- Exhibits effective verbal and written communication skills
- Performs other job-related duties as assigned
Requirements:
· Registered Nurse, BSN required, MSN preferred
· Minimum 2 years clinical quality experience
. CPHQ certification preferred
