Registered Nurse, Assertive Community Treatment (ACT)

As an ACT Registered Nurse, you will assess patients with mental disorders and provide medical care and medication management for patients in your care. You will be responsible for ensuring that patients have access to basic preventative care and treatment of mental health and medical needs. You will report to the Director of ACT, Licensed Professional, and work onsite and in the community.
Responsibilities:
- Maintain clinical records on all patients seen in a manner consistent with confidentiality of health care records guidelines and department regulations protocol.
- Support treatment planning and medication needs of all clients.
- Administer injectable medications for clients enrolled in the ACT program, overseen by the Nurse Practitioner and Director.
- Conduct face-to-face assessments with client and collaterals and maintain corresponding clinical documentation pertaining to these sessions/assessments.
- Serve as the point of contact for primary care providers, pharmacies and additional medical service providers ensuring the patient is receiving appropriate care.
- Work in collaboration with the Nurse Practitioner and Licensed Practical Nurse for the ACT program as needed in regards to patient care and treatment.
- Participate in daily team meetings.
- Compile a routine medical inventory checklist, staying in communication with the Director on program’s medical needs.
- Lead in the organization of maintaining medications, creating medication boxes, and aid in medication distribution.
- Other duties as assigned.
Requirements and Education:
- An individual currently licensed to practice as a registered professional nurse issued by NYSED
- 2+ years of nursing experience in psychiatric, pediatric, or community health settings
- Valid NYS driver's license required
