We are seeking compassionate and driven Nurse Practitioners to join our dynamic Clover Home Care teams across New Jersey and Georgia. This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of some of our most vulnerable members, helping to improve health outcomes and quality of life.

As a Clover Home Care Nurse Practitioner, you’ll join one of two specialized care teams, each playing a vital role in improving outcomes and delivering personalized care:

  • Clover Care Visit (CCV) Team: The CCV team focuses on early intervention and transitional care, helping to set the foundation for long-term health and reduce avoidable complications. You’ll work closely with our clinical insight platform, Clover Assistant, to support decision-making and improve outcomes.
    • Conduct comprehensive in-home health assessments for enrolled Medicare Advantage members.
    • Perform post-discharge follow-up visits to reduce hospital readmissions.
    • Collaborate with interdisciplinary teams using data-driven clinical tools.
    • Ideal for Nurse Practitioners who are proactive, skilled in transitional care, and confident using technology and clinical data to guide care.
  • In-Home Care (IHC) Team: The IHC team provides longitudinal primary care for members with complex and chronic conditions, focusing on continuity, quality, and outcomes. Clinicians on this team have a high degree of autonomy and play a central role in ongoing care planning.
    • Manage a dedicated patient panel, delivering ongoing in-home primary care.
    • Care for members with chronic illnesses and advanced medical needs.
    • Participate in a shared on-call rotation to ensure continuous support.
    • Best suited for Nurse Practitioners who value clinical autonomy, excel in building lasting patient relationships, and are skilled in managing medically complex cases throughout the continuum of care.

You should get in touch if:

  • You have an active and unencumbered NJ Registered Nurse license or GA Registered Nurse license.
  • You are a Certified Advanced Practice Nurse Practitioner (APN).
  • You have a master’s degree or post-master’s certificate in advanced clinical practice.
  • You have a minimum of 1 year of NP experience in a clinical setting.
  • You have active DEA and CDS registration.
  • You have current Basic Life Support (BLS) certification.
  • You are based in New Jersey or Georgia and able to travel locally for home visits.
  • You have experience in home care or house call settings (preferred, but not required).

A reasonable estimate of the base salary range for this role is $140,000 to $165,000. Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
