How you’ll contribute
As a Nurse Navigator (RN), you will serve as a patient's guide through the healthcare system, helping coordinate care, remove barriers, and improve the overall patient experience.
Job Duties
- Serve as the primary point of contact for patients and families
- Coordinate appointments, testing, treatments, and referrals
- Provide education about diagnoses, treatment options, and available resources
- Assess barriers to care and connect patients with community resources and support services
- Advocate for patients and helping ensure timely, appropriate care across the continuum
- Provide hands-on patient care including but not limited to vitals, patient intake, in-clinic procedures
- Cover other clinics as necessary
- Other duties as assigned

Qualifications and requirements
- Strong clinical judgment and critical thinking.
- Ability to work independently in a fast-paced setting.
- Calm, confident decision-making under pressure.
- Clear communication with patients and care teams.
Qualifications
- Standard ADN and BSN preferred
- Outpatient Clinic preferred
- ACLS and BCLS certifications required.

- BCLS (BLS) certification
- Associate Degree in Nursing (ADN)
- Bachelor of Science in Nursing (BSN)
- Outpatient clinic experience
