Job Summary: The Nurse Navigator's primary function is to serve as first contact for patients with positive breast cancer diagnoses, and guide patients and families through the health care system from the time of diagnosis through their duration of care, by assisting them with access to needed services, developing relationships with various providers and tracking interventions and outcomes.
Coordinates this care to ensure safe, effective, efficient patient centered care. Serves as a central resource to patients and families to ensure exemplary care is provided. Communicates effectively with the Radiology & Surgery personnel and other department personnel.

Job Description:

Essential Responsibilities:
  1. In an effective manner, initiates communication with patients regarding pathology results and follow up plan for abnormal results . Guide patients through the health care system. Assists patients in connecting with community and social support services. Facilitates interaction and communication with health care staff and providers.
  2. Provides breast health education to individuals and groups. Develops and maintains a learning library for patients that include language specific materials.
  3. Identifies personnel in departments involved in the care of breast health patients (i.e., physicians, nurses, radiology, social services, radiation oncology & hematology/oncology) and develop relationships with them.
  4. Assist/direct patients with any questions/ concerns regarding payment of services. Assists with arranging for patient transportation where needed. Builds relationships with other patient navigators.
  5. Tracks interventions and outcomes. Through the use of formal screening performed at the time screening imaging, identifies patients at a higher risk for follow up consultation.
Required Qualifications:
  1. Bachelor's degree in Nursing required.
  2. License Registered Nurse required., and Certificate 1 Basic Life Support required.
  3. 3-5 years related work experience required in caring for surgical patients.
  4. Strong analytic skills with quality and data analysis.
  5. Excellent interpersonal skills. Ability to work with a variety of personalities and disciplines.
  6. Experience with interdisciplinary health care approach.
  7. Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.
Preferred Qualifications:
  1. Oncology experience.

  5. Oral Communications: Ability to comprehend and communicate complex verbal information in English to medical center staff, patients, families and external customers.
  6. Knowledge: Ability to demonstrate in-depth knowledge of concepts, practices and policies with the ability to use them in complex varied situations.
  7. Team Work: Ability to act as a team leader for small projects or work groups, creating a collaborative and respectful team environment and improving workflows. Results may impact the operations of one or more departments.
  8. Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s). Resolves service issues in the assigned unit(s) in a timely and respectful manner.
Age based Competencies:
Employees in this job must be competent to provide patient care to the following age groups: Neonatal:Birth to 6 months, Youth: 6 months to 16 years, Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 -.
Physical Nature of the Job:
Heavy work: Exerting up to 100 pounds of force occasionally and/or 50 pounds of force frequently. Work is physically demanding and requires extended periods of standing and/or moving.

Pay Range:

$37.08 - $92.84

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
