Responsibilities:
- Act as a primary case manager to 3-6 consumers
- Provide nursing interventions to individuals
- Facilitate and coordinate medical care to include communication with health care providers
- Provide health education to individuals
- Assist in the management of medications and the overall functioning of the nursing department
- Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload
- Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
- Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients
Qualifications:
- Requires an associates degree in nursing
- RN nursing license
- Experience working with adults with mental illness and knowledge of psychiatric and medical medications
- Valid Massachusetts drivers license
