Phoenix House is seeking a Licensed Practical Nurse to support clients' health, safety, and recovery in a behavioral health and substance use treatment setting. This role is ideal for an experienced LPN who understands the importance of compassionate care, clinical skill, and thoughtful client engagement.
As an LPN, you will be a trusted member of the care team, helping clients navigate health-related needs during a critical stage in their recovery journey. You will assist with medication support, monitor client health concerns, document care, coordinate medical follow-up, and collaborate closely with clinical and program leadership to support safe, effective, and compassionate care.

The Licensed Practical Nurse will perform the following tasks:

Provide direct nursing care to clients under the supervision and direction of clinical leadership.

Maintain patient confidentiality in accordance with Phoenix House policies and procedures.

Oversee unlicensed assistive personnel (e.g., CNAs, Medical Assistants, and Medical Technicians).

Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies.

Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.

Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.

Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies.

Other duties as assigned.

REQUIRED EDUCATION

- At least three years of related experience
