IOA's Nurse Liaison provides education and training to RCFE’s in network; serves as content expert for waived services programs; serves as primary liaison with the Health Plan team; works collaboratively with Director, Social Workers, and Supervisors to ensure quality measures are completed.
This role will provide eight hours of support per week.
RESPONSIBILITIES:
  • Act as consultant for Care Management staff regarding medical health issues individuals are facing such as diabetes management, medication management, wound care, and other acute or chronic illnesses.
  • Participate in multidisciplinary team meetings and care conferences; provide recommendations regarding safe level of care in conjunction with multidisciplinary team.
  • Ensure individuals transitioning out of institutional settings have long-term supports and services necessary to promote safe community living.
  • Review all discharge paperwork and consult as needed with inpatient, outpatient, and primary care providers
  • Provide ongoing trainings to CalAIM staff relevant to client population and program.
  • Complete required assessments and documentation for waived services programs. Liaise with Care Management staff to ensure clinical documentation meets best practice guidelines.
  • Review utilization patterns of individuals with high health care utilization to analyze medical factors leading to utilization and assist Care Management staff to implement interventions regarding health promotion activities.
  • Works collaboratively with CalAIM Leadership Team to improve program and process efficiencies.
  • Monitors the quantity and quality of the services provided by other involved providers.
  • Participate in and promotes ongoing efforts towards Continuous Quality Improvement.
  • Attends and actively participates in team and program meetings, activities and problem-solving endeavors; contributes to open lines of communication within the team.
  • Understands and applies the regulatory and procedural requirements of the Institute on Aging.
  • Attends continuing education classes and/or in service training to increase knowledge, skills and attitudes related to CalAIM client population.
  • All other reasonably related responsibilities as assigned.
EDUCATION:
  • Registered Nurse (R.N.) license in California required.
     
BACKGROUND AND EXPERIENCE:
  • Minimum two years working with disabled adults and/or older adults as a registered nurse required.
  • Experience providing trainings and education to a diverse group required.
  • Excellent verbal and written communication skills required.
  • Experience working with individuals with mental and/or behavioral health diagnoses andsubstance abuse disorders highly preferred.
  • Prior experience working with a health plan and/or in a SNF/Hospital setting preferred.
  • Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks.
  • Computer literacy required.

COMPENSATION:

Range: $53 to $56/hour

This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.
