About This Role:
- Care Coordination & Case Management
- Coordinate care for patients and families by connecting them with providers, specialists, behavioral health services, and community resources.
- Develop, monitor, and update individualized care plans to support patient-centered goals and improve health outcomes.
- Patient Education & Engagement
- Provide individual and group education on behavioral health, chronic conditions, healthy behaviors, and available Evara Health services.
- Engage patients and families in their care, helping them navigate services and overcome barriers to treatment.
- Population Health & Quality Improvement
- Identify high-risk patients, monitor progress, and collaborate with care teams to ensure patients receive the right level of care at the right time.
- Assess service utilization, identify gaps in care, and support initiatives that improve quality, continuity, and patient outcomes.
- Collaboration & Team Support
- Partner closely with providers, therapists, nurses, and care team members to coordinate comprehensive patient care.
- Participate in huddles, case reviews, committees, and workflow improvement initiatives while serving as a resource for staff and community partners.
- Documentation & Compliance
- Maintain accurate documentation, manage assigned tasks, and ensure timely follow-up in accordance with Evara Health policies.
- Deliver exceptional customer service while adhering to all HIPAA, State, Federal, and organizational requirements.

Education & Experience:
- Graduate of an accredited RN Program
- Valid and active RN license in the state of Florida
- Required: Minimum two (2) years as an RN in a healthcare setting with some supervisory experience
- Preferred: Minimum two (2) years working with pediatric population
- Preferred: Certified Case Manager
