Job DescriptionThe Clinical Supervisor is directly responsible for clinical activities of the field staff. Plans, implements and evaluates patient’s plans of care for appropriateness to individual patient’s needs. Plans for and assists in conducting team conferences, shares in the responsibility of assessing clinical case management activities.
Performance Standards:
1. Assures that initial in-home evaluation of any patient requiring the service of a registered nurse, licensed practical nurse, home health aide or other nursing support staff is performed by a registered nurse
2. Enforces and demonstrates compliance with Federal, State, Local and accreditation bodies regulations and standards as well as Maxim policies at all times
3. Assess and supervise the delivery of nursing services
4. Prepares and coordinates clinical notes and progress notes
5. Provides in home supervision of nursing services delivered by licensed nurses, home health aides, and other nursing support staff
6. Reports changes in the patients’ medical or mental condition to the attending physician and the Director of Clinical Services
7. Facilitate and promotes patient and family or significant other in education to assist and promote return of optimal health status
8. Participates in the continuing review of service and participation in the performance improvement program
Qualifications1. Graduate of an accredited school of nursing, BSN preferred
2. Currently licensed as a Registered Nurse in the state
3. Homecare Experience strongly preferred
4. Trac, G-Tube, Case Management Expereince strongly preferred
5. Demonstrated ability to supervise and direct professional administrative personnel
6. One year prior professional nursing experience
Additional InformationShift Specifics: Monday-Friday 8am-5pm, some flexibility to schedule! 40 hours weekly, guaranteed.
Compensation: 60K yearly, plus bonuses! Great clinical team!
