Provide evidence-based nutrition assessment and counseling to individuals and groups to support health outcomes. Collaborate with interdisciplinary teams to develop and monitor nutrition care plans.

Main Duties- Conduct nutritional assessments using medical history, dietary intake, anthropometric measurements and relevant clinical data.
- Develop and implement individualized nutrition care plans and therapeutic diet recommendations.
- Provide one-on-one and group nutrition counseling and education for clients, families and caregivers.
- Monitor client progress, evaluate outcomes and adjust interventions based on clinical indicators.
- Collaborate with physicians, nurses and other health professionals to integrate nutrition into overall care plans.
- Document client interactions, prepare reports and contribute to program development, quality improvement and community outreach.
- Bachelor's degree in dietetics, nutrition science or a closely related field.
- Completion of an accredited practicum or internship in dietetics or equivalent supervised practical experience.
- Registration or eligibility for registration with the provincial regulatory body for dietitians (or equivalent) as required by jurisdiction.
- Experience conducting nutritional assessments and developing care plans in clinical, community or public health settings.
- Ability to work collaboratively in interdisciplinary teams and manage multiple caseloads effectively.
- Proficiency with electronic health records, nutrition analysis tools and common office software.
