What You'll Do

As the MDS RN Coordinator, you'll help ensure each resident receives the individualized care they deserve by working closely with our interdisciplinary team.

Responsibilities include:

  • Coordinating and completing MDS assessments.
  • Leading the interdisciplinary care planning process.
  • Partnering with nursing, therapy, dietary, social services, and physicians.
  • Monitoring assessment schedules and regulatory compliance.
  • Assisting with admissions, evaluations, and care transitions.
  • Supporting quality improvement initiatives.
  • Serving as a resource for reimbursement and assessment processes.

  • Holds a current RN license.
