As the MDS RN Coordinator, you'll help ensure each resident receives the individualized care they deserve by working closely with our interdisciplinary team.
Responsibilities include:
- Coordinating and completing MDS assessments.
- Leading the interdisciplinary care planning process.
- Partnering with nursing, therapy, dietary, social services, and physicians.
- Monitoring assessment schedules and regulatory compliance.
- Assisting with admissions, evaluations, and care transitions.
- Supporting quality improvement initiatives.
- Serving as a resource for reimbursement and assessment processes.
- Holds a current RN license.
- Has strong assessment and critical thinking skills.
- Communicates well with residents, families, and coworkers.
- Enjoys collaborating with others.
- Is organized and detail-oriented.
